About Your Visit
If you are a new patient, please arrive to your appointment early in order to complete the registration process. In addition, please bring the following:
- Driver's License or State ID
- Referral (If Required by Insurer)
- Insurance Identification card
- Copies of Operation Records, Medical Records, X-rays, MRIs, and CT Scans from Prior Doctor Visits
- A List of Current Medications and Any Known Allergies
Currently we are in network with Medicare, Cencal, Blue Shield Policies with Cottage Hospital Employees, and most Workers Compensation plans. We regularly treat out of network patients and strive to make your care affordable. As a courtesy, we will bill your insurance. Payment plans can be arranged.
Please call our office to make an appointment today and we will do our best to accommodate. If you are unable to keep your appointment at the scheduled time, please contact our office so we can re-schedule the appointment for a time that works better for you.
Fees & Payments
We accept cash, checks, MasterCard and Visa for your convenience. We will complementary provide insurance billing for services rendered. We will help in any way we can to assist you in handling claims.
